Generations Home Health Care

Generations Home Care offers a full range of skilled nursing services as well as physical and occupational therapy that are delivered in the comfort and privacy of the patient’s home. Every patient receives a personalized plan of care that is directed by the patient’s physician.

Home health care is available on a short- or long-term basis and can be a good choice for patients with many different types of needs. Our services can assist patients through the recovery process as they heal from surgery or injury. They can also provide help for individuals who are living with challenges of aging such as loss of memory or decision-making skills, difficulty maintaining balance or inability to perform daily personal care tasks or household duties.

Skilled Nursing Care

Working together with the patient’s physician and family caregivers, Generations’ registered nurses can provide a wide range of medical services in the home following a standardized disease management clinical pathway, including:

These services play an important role in helping patients and their families manage the effects and symptoms of life-changing medical conditions such as Alzheimer’s disease and dementia, cancer, heart disease and diabetes.

Physical Therapy

Physical therapy can help patients maintain their independence by regaining mobility. Those who can benefit from physical therapy include individuals who have suffered a stroke, are recovering from bone fracture or joint replacement surgery or have a chronic condition that interferes with mobility, such as Parkinson’s disease.

Generations’ certified physical therapists work with patients to improve strength, coordination, range of motion and balance. Therapies are designed to help patients:

Occupational Therapy

Often patients who are recovering from stroke, illness or other injury find it more difficult to perform daily activities that were once routine – tasks such as eating, bathing, grooming and dressing. Occupational therapy supports these patients in their desire to remain living independently at home by helping them regain the ability to perform these tasks.

Generations’ occupational therapists come to the patient’s home, helping them improve motor skills necessary to perform such activities as dressing and eating. These therapists can also provide equipment and teach the patient techniques that make it easier and safer to perform these tasks with their changing abilities.

Personal Care Services

Generations’ certified home health aides assist patients with the daily tasks that are necessary to maintaining their independence and remaining in their own homes.

Through our personal care services, patients receive help with everyday tasks including:

These services can be provided regardless of whether the patient requires in-home medical care. We work together with our patients’ families to determine a schedule and program of services that best meets their needs.
