Intake Process and Plan of Care
Once a referral is made, the patient is assigned to a nurse case manager. Within 48 hours of receiving the referral, the case manager will make a home visit and perform a comprehensive assessment of the patient’s health, safety and daily needs, including:
- Physical condition and wellness
- Sensory, neuro/cognitive and psychosocial function
- Ability to perform activities of daily living
- Nutrition
- Medication use and compliance
- Assessment of family caregivers’ ability to assist with care
- Home environment
Based upon the findings of these assessments, the nurse develops a personalized plan of care tailored to the patient’s specific needs and situation, conferring with the physician to determine the services and frequency of visits needed.
Designed to ensure that all team members are working successfully in collaboration to provide continuity of care for the patient, this plan can include:
- Description of the patient’s condition and reasons why home care is needed
- Long- and short-term goals for treatment with expected outcomes
- Assessment of each person’s responsibility in the patient’s care
- Teaching packet for family caregivers
- List of necessary medical equipment and supplies
